PERAN UREUM DAN EGFR DALAM MENENTUKAN KADAR HEMOGLOBIN PADA PASIEN PENYAKIT GINJAL KRONIK NON-DIALISIS
Abstract
Penyakit ginjal kronik (PGK) merupakan masalah kesehatan global yang ditandai dengan penurunan laju filtrasi glomerulus (eGFR) dan sering disertai anemia sebagai komplikasi utama. Penelitian ini bertujuan menganalisis hubungan kadar ureum dan eGFR dengan kadar hemoglobin pada pasien PGK non-dialisis di RSUD Prof. Dr. H. Aloei Saboe, Gorontalo. Penelitian ini menggunakan desain analitik kuantitatif dengan pendekatan potong lintang retrospektif berdasarkan data rekam medis tahun 2024. Sampel sebanyak 52 pasien diperoleh melalui teknik purposive sampling. Analisis dilakukan secara univariat dan bivariat dengan uji normalitas Kolmogorov–Smirnov serta uji korelasi. Kebaruan penelitian ini terletak pada analisis simultan ureum dan eGFR berbasis formula CKD-EPI 2021 dalam hubungannya dengan hemoglobin pada populasi PGK non-dialisis di tingkat rumah sakit daerah yang masih terbatas diteliti. Hasil menunjukkan seluruh pasien memiliki ureum tinggi, eGFR rendah, dan hemoglobin di bawah normal. Terdapat hubungan bermakna antara ureum dan hemoglobin (arah negatif) serta eGFR dan hemoglobin (arah positif). Disimpulkan bahwa peningkatan ureum dan penurunan eGFR berhubungan dengan rendahnya kadar hemoglobin. Temuan ini menegaskan pentingnya pemantauan anemia bersamaan dengan fungsi ginjal dalam pengelolaan PGK non-dialisis.
